Letter of Non-Attendance Request

Instructions

This form is intended to be used to request a letter from EKU indicating that you did NOT attend and earn grades. The letter will be sent electronically to the recipient's email address you list below. If you did enroll and earn credits at EKU, you will need to instead request an official transcript here.

Note: if you are attempting to withdraw from EKU you would need to complete the Non-Participation Form here.

Please note that the recipient email cannot be the student email. It must be the employer, educational institute, or employer that is requesting a verification of non-attendance. All submissions with the recipient email the same as the student email will be denied.
